It is time to complete your annual extension ministry forms per ¶343 and ¶344 of the 2020/2024 Book of Discipline. These materials assist the Cabinet in fulfilling its supervisory responsibilities and in discerning your appointment for the coming conference year. Failure to submit the required extension ministry reports will initiate a full conference relationship review.

All required forms are available on the Conference website: www.susumc.org/extension-ministry/ . Per ¶344.3, please include the name and address of your charge conference; and a copy of your charge conference report alongside your extension ministry forms.

If, through prayerful discernment, you feel called to receive a local church appointment for 2027–2028, or if you anticipate retirement, please submit your request or notice by December 1, 2026 to the cabinet administrator at cabinetadministrator@susumc.org, with a copy to your superintendent.

Please ensure that all fields in the Appointment to Extension Ministry Form are completed. Follow the instructions carefully regarding any additional documentation required. If corrections are necessary, clearly note and highlight those changes. Include your conference email address, as all official communications are sent exclusively through that account.

Submit either electronic or hard copies of your completed report to the individuals listed below by January 1, 2027.

3. Chairperson of Conference Relations – Rev. Stephanie Rupert
4. Bishop of the Area in which you work (If not Susquehanna Conference)
