·         NEW this year

o   Form 9b – Weekday Children’s Ministries Report.  This report is to be filed if you answer “yes” to any of the ministries in Form 9, #11 - weekday ministry such as Daycare, Pre-school, Nursery or similar ministries. 

 

ALL FORMS  must be sent to the District Office at least TWO WEEKS prior to your charge conference!  If there is any additional business to be conducted, that must also be received by the district office 2 weeks prior to your charge conference.

 

Charge Name:  Please check the conference journal for the correct name of the charge.

o   Example:  You may think you serve First UM Church, Mongolia.  However, due to necessary computer sorting features, the Charge name is Mongolia, First.   The Church name is First UMC.  Check the Journal!

·         Form 4 –you will find form 4a, 4b and 4c.  These are for Local Church Officers, Charge Officers and Churches using the Alternative Minimum Structure.

o   For single point charges, fill out Form 4a. 

o   For Multi-point charges, fill out form 4a for each church, leaving SPRC and Lay Members blank.  Then fill in Form 4b with all Charge officers.

o   Form 4c is for churches using the alternative minimum structure

§  List all council members including the “class” they are in (they should be in three year classes)

§  List their designated role – Council Chair, Lay Leader, Lay Member, Trustee Chair, Trustee vice chair, Trustee secretary, 2 members for SPRC concerns and 2 for finance

·         Some persons may have 2 designations (Lay Leader may be one of the SPRC people – designate both roles),

·         those who may not have a specific role should be listed as “council members”

§  List other church staff/officers as applicable
